From Images to Insight: Measuring Pathology in People Living with Multiple Sclerosis
Magnetic resonance imaging (MRI) is central to diagnosing and monitoring multiple sclerosis (MS) and has helped develop effective therapies that reduce relapses and new lesions. However, many people with MS-especially those with progressive MS-continue to worsen even when inflammatory activity is well controlled. This highlights the need for MRI markers that reflect disease processes related to progression rather than relapses alone.
This presentation introduces emerging MRI techniques that measure tissue damage and repair beyond conventional lesion count and volume measures. These include imaging markers of remyelination, cortical demyelination, and chronic active (“smoldering”) lesions, which are associated with ongoing tissue injury and disability progression. In addition, imaging of the choroid plexus suggests a possible role for blood-CSF barrier dysfunction in diffuse brain injury.
Together, these approaches provide new insights into why disability accumulates in MS and offer promising tools for studying progression and evaluating future treatments.
